Support coordination is about supporting you in building your capacity to find suitable providers, navigate choices, understanding service agreements and schedule of fees from providers and ensuring access to NDIS documentation and supports in your plan.

Our Service to you
We Care Providers offers Support Coordination service for participants who want to fully maximise their NDIS plan. This service gives the participants the autonomy to choose over the supports that they need and tailor-fit each support to their requirement.


How can We Care Providers Support Coordinator assist you:
Once you are funded for NDIS Support Coordination, Support Coordinator will connect you with various supports and services in the community and in the Government as per your NDIS Plan. Our role is to help you put your NDIS Plan into action and to the optimal level.

Our Support Coordinators will:
- Help you understand funding and services that are made available in your NDIS Plan
- Help you understand how you can use your NDIS plan to meet your goals
- Help you understand how to use the NDIS portal so you can monitor how your funds are being utilised
- Introduce you to support options and providers and help you choose your preferred support/provider
- Negotiate your preferred services and prices, as well as develop service agreements and create service bookings with preferred providers
- Work with your circle of supports (family, GP, therapists, friends) in order to meet your goals of your individualised NDIS Plan
- Monitor the progress of your services and outcome of achieving your goals
- Assist you in preparing for your next NDIS review meeting
